Well, which are you? Were you born in England? Were you naturalized? Were your parents Brirtish citizens, but were born on foreign soil?

I think it's Foreign Birth Registration.

I'm assuming you were born in the UK but are claiming an Irish passport 'cos of your dad's nationality. If in doubt, isn't there a phone number on the paperwork you can call?

You need Irish citizenship, I think. It's asking you how you received citizenship (naturalization process, ex. moving and taking the exam; foreign birth to Irish parents; through marriage).

Isn't there an Ireland immigration site that can answer this?

dont they normally come with a little booklet that gives you advice about each section??

or you could ask in the Post Office (im guessing thats where the application came from)

I'm just surprised. Most people don't start filling in all that sort of bureaucraziness until they're like, 18, 19, 20, whatever.

Yeah, that would be naturalization then if they allow the children of parent(s) who STILL have Irish citizenship to apply for a passport. Make sure that Ireland lets children apply if only their mother has citizenship, because sometimes countries only allow children to apply if their father or both have citizenship.
