I am gay and pretty much out to everyone but my grandparents.

The thing is they are not even my biological grandparents (but I love them as if they are) and they are pretty much the only people who don't know, but I am just so worried about it. It's confusing because her real son is gay and she didn't talk to him for a while, is also against gay marriage. She's fine with him now though.

Do I tell them? My biggest worry is my grandma stop talking to me if I do, even for a little while

If she came around for her son maybe she will be more accepting of you. Have you talked to her son about it? Maybe he has a better feel for where her head is on the subject.
