I think it's actually one of the more well-known fetishes out there; not quite on the same level as feet or leather, but it's known. Weird? Maybe, but even vanilla sex is weird when you think about objectively.

Anything non-fetishistic. (Although a fetish is usually described as arousal caused by anything other than vanilla sex, so that's not a particularly useful definition. :-/) I would say anything that only involves the body's usual erogenous zones is vanilla. But that's my opinion; there are probably people who would say that only standard missionary position is vanilla, and anything else is kinky. Different strokes for different folks, I suppose.
