It doesn't if you are doing it properly; i.e. a little 'loosening up' for a few minutes before hand with something smaller (fingers work nicely), and lubricant and there shouldn't be any pain at all. Lubricant is critical, and not using it (or not using enough) is usually the main cause of pain.
