It's just a natural sound that kind of comes out sometimes. It's more of a sound of pleasure, not pain (unless it sounds a specific way that indicates pain). I moan during masturbation sometimes. It doesn't hurt, just feels good, and the intensity of pleasure causes that sound to release. For a person who is being penetrated (man or woman) It's easier to let out noise because of the feeling of being "filled". On the other hand, some people in porn make fake noises to please the audience who might have that kind of fetish. For me, It's easy to tell the difference between fake vs natural.
Its something that I expect to happen because its all I know. All of my partners have been into moaning and groaning and even lots of dirty talk which I love. People who moan and make noise when aroused are a turn on for me. Take your typical on-line porn clip, for example. About 50 to 60% of the time, no one makes a sound, which could be for a variety of reasons. Perhaps it's some union work rule, requiring extra payment for some noise. Or maybe it's some producer in the Czech Republic trying (vainly I might add) to mask the Slavic origins of the content. Or maybe because they're bored and not sure there will be payment at the end.
