I do know in Dragon Age Origins if your character is male he can be involved in a homosexual relationship with a gay elf.

Also in Mass Effect I think the same is true for a lesbian relationship.

I don't think you can directly play as him but Grand Theft Auto's Ballad of Gay Tony has you playing as his bodyguard so he's closely intertwined with your character.

Also (since I seem to be in a Rockstar mood today) the game Bully lets you kiss both guys and girls (except you can only kiss one guy from each clique but they tend to just appear if you know where to find them ^_^).

Apparently you can get a same-sex marriage in Skyrim, but I haven't seen it myself yet.

Dragon age would be the main example of a game that totally supports it, with specially written dialogue and everything (aka: the gay romance is not just using the dialogue for the straight romance with the pronouns changed).

Dragon Age 2 then expanded on it by giving even more options for both romances or sleeping around (had I not been comitted to my dear Fenris, I would have totally gone for a quick hookup with Zevran during his cameo scene).

Main drawback to the DA romances is that they tend to end in horribly dramatic ways. there's more ways of tragically ending them than to live happily ever after.

Mass effect had a kind of lesbian relationship, though I'm still not sure whether interspecier relationships with Asari (who look female, but are an agendered species) count as lesbian. Mass Effect 3 will have gay options though, last I heard...

Fallout New Vegas takes Fallout 3's early gender-based perks (Black Widow and Ladykiller, both of which gave damage bonuses and dialog options for the opposite sex) and added same-sex options (Cherchez la Femme for the ladies and Confirmed Bachelor for the men).

Making a gay male is interesting as now you have magical gaydar that lets you know when you can hit on Manny Vargas.

Or, you can take both perks and make your character bi for a 5% damage bonus to everyone.

In Dragon Age, I hooked up with Zevron for a bit... but I play a goodie-two-shoes type character, so he and the rest of my party never got along well enough for our relationship to continue. He was really my rebound though... Alistair was quite straight, unfortunately, and never fell for my witty wily charm

Fable romances were just ridiculous... vapid and pointless

But, I would love to play a game where the protagonist was homosexual (and not just could "choose" to be during actions or at character creation). A lot of the wiki games had NPCs, or only brief cameo player characters, that were homosexual, which doesn't really count for vicarious gratification
